I'm thinking he gets the cure and lives. I don't see them killing him off. Whatever happens to Roy probably happens to Slade, allowing Slade to return at a later date (perhaps if/when they introduce Jericho).
I think he gets the cure and survives to cause trouble another day; perhaps Waller will take him into custody and offer him to join the Suicide Squad instead of rotting in prison. I don't see Slade dying this early in the series, he's a huge nemesis for GA in the comics. If Malcolm survived past
I think he'll get the cure and live- I feel like we're going with a parallel with opposite results with present day-flashback. In the flashback, Oliver wanted to cure Slade, but ended up 'killing him', while in present day, Oliver wants to kill Slade but ends up curing him. I think he'll get locked up, which means he can potentially show up again in the future either by escaping prison or joining the suicide squad.
I think he will get the "cure" and live, though I don't think the cure will work on him as with others. He's too much mixed with the mirakuru. But we will see him more like the comics.
I hope Slade doesn't kill Sara and think he will be forced with the cure and go away, either to jail or to regroup and become a threat again in the future.
Him getting cured and locked away, maybe Amanda putting him on the Suicide Squad is the only fitting end to the character, and the most fitting to Oliver's path of not being a murderer.

He gets the Cure, he lives and he was taken prisoner by Amanda. In season 3, he joined the Suicide Squad.
