    So when dean looked in the mirror his reflection of his face looked fucked up but then his face in real like looking at the reflection appeared intact, so my guess is yes gesturing into a demon.. That representing his soul changing

    I think that should have been an obvious route that was going to happen anyways. Cain becomes a Knight of Hell because of the Mark of Cain…Dean himself could be turning into a Knight of Hell.

    No, he was the first KoH.

    This is what Cain said in "First Born":

    "Abel wasn't talking to God. He was talking to Lucifer. Lucifer was gonna make my brother into his pet, I couldn't bear to watch him be corrupted, so I offered a deal — Abel's soul in Heaven for my soul in Hell. Lucifer accepted... as long as I was the one who sent Abel to Heaven. So, I killed him. Became a soldier of Hell — a knight."

    He looked fine to me. Maybe a little cut up or something but I did NOT get any impressions from that scene that he is turning into a demon. I could be wrong and they could still move the plot in that direction but he didn't look like he was turning darkside in the mirror reflection.
