I believe Sherlock was investigating how in ancient times people believed that making holes in the heads of those infested by a devil or evil spirit could let it out. The relevance of this is beyond me at the moment. Hopefully it becomes clearer in future episodes
decent ep, not one of the stronger ones for me, but I really liked the progression and development of Sherlock as an addiction sponsor for Randy showing some real growth for the character
