Or, Dean is "connected" to both Sam and Ezekiel and Ezekiel "senses" (whatever) Dean floundering in his trying to convince Sam to live and seamlessly "steps in" as "Dean". Ezekiel will have a vested interest in 'saving Sam' at any cost because Dean has promised something, IDK what at the moment. I didn't expect this, but good twist.
more like Ezekiel either pretended to be Dean inside Sam's head or more likely when Sam agrees to live Ezekiel goes into Sam's head via Dean to 'heal' him. When I say 'heal' I mean possess because that's what all the signs are pointing to.

my guess is Ezekiel only steps in when Sam agrees to live, we see Dean's image inside Sam's head morph into the angel who has his hand on Sam to 'heal' him
Great idea and if this happens the way you described it would some of the onus off Dean for allowing his brother to be 'noncon' angel-possessed; Sam agrees to live but doesn't realize at first (because of near-death amnesia) how/why he's (Sam) been 'healed'.

Maybe when the angels lost their wings they also lost their full "resurrection powers"; i.e, Sam dies and then Ezekiel resurrects him easy peasy unless Death refuses to allow Ezekiel to resurrect Sam. I just think that the favor is something else other than Dean 'allowing' Ezekiel to use Sam's bod temporarily as vessel. It's clear to me that DoorNail!Sam =/= No Dean Favor; I don't see what Ezekiel would gain if the favor was temporary control over Sam's body, except hiding in a Winchester. (As a total aside, I still don't know how Crowley will not immediately "see" Ezekiel in Sam).
my guess is Ezekiel also wants help from the brothers in getting the angels back into heaven, there are other angels that have been mentioned in spoilers that would take up the role of take over earth or go dark-side or just live human so maybe Ezekiel is one who wants to restore everything to how it should be? Carver is going to have to make the Winchesters part of the central angel story line and this would tie in quite nicely imo.
That' s exactly in line with what I thought "Dean's Favor" would be - the Winchesters have to find a way to get Ezekiel and the other angels back home and to hold up Zeke's end, Zeke has to have Sam live or no dice. I agree that this would give not only Castiel (unlike S6) but the Winchesters, integral involvement in a major mytharc this season - finding a way to reverse Metatron's spell and the damage it will cause 'us '(humanity) powerful, but wingless, angels are allowed to move unimpeded on earth. We already know that Abaddon is building a demon army with actual soldiers and is going to demanding that angels bow to her also. What happens if some of them want to join her? And Malachi (9.09) seems to be intent on starting his own faction. What if these two groups of angels - one lead by Abaddon, one by Malachi - decide they "prefer to live here" (the premiere title IS "I think I'm going to like it here") and wage war between them - with the prize subjugating humanity?The Winchesters/Castiel can't count on all the angels *wanting* to back to Heaven.
