I think House is strangulate the patient because he said something about Wilson and house basically do that and says try to stay alive while you slowly die....due to Wilson slowly will die of his cancer...
My guess is the patient and Wilson are reluctant to do treatment that will prolong their lives. House, being upset about Wilson's decision, flips out and chokes the patient to make him fight for his life to prove that he really doesn't want to die. The still of that scene says "House tries to prove a point to his patient." And that's the only point I figure he could be trying to make.
It's possible he's doing something medical, but I'm not sure what point that would be making. I watched the promo in slow motion and there is one frame where it looks like House is using an ultrasound wand on the guys neck, which makes it far less likely he's choking him. Although, I don't see what he would be gaining in the situation by just not sterilizing his neck especially since his apparant point is to keep the guy alive. I still think that if he's not choking him, he's forcing a life-saving treatment the patient didn't want.
Right, the tracheotomy. Forcing the patient to accept a treatment he doesn't want. Just like he wishes Wilson will accept more chemotherapy that he doesn't want. Should be a great episode.
